This course emphasizes practical approaches to gaining skills and confidence in leadership roles.

    Foundations of Chronic Disease Management
    (30 hours)
    This course leads to a foundational understanding of the field of chronic disease management, including prevalence of diseases and current models of management.

    Supporting Client Self Management
    (30 hours)
    This course introduces the LPN to the skills and understandings needed
    to support clients in their self-management: healthy living, lifestyles,
    change, and motivation.

    Self Management of the Respiratory Chronic Diseases: Chronic Obstructive Pulmonary Disease (COPD) and Asthma
    (30 hours)
    This course enhances the LPN knowledge of the pathology, assessments, associated diagnostic tests, current treatments, pharmacological management, and the specifics of supporting client self- management of COPD and asthma.

    Self Management of the Cardiovascular Chronic Diseases: Hypertension, Hyperlipidemia and Congestive Heart Failure (30 hours)
    This course enhances the LPN knowledge of the pathology, assessments, associated diagnostic tests, pharmacological management current treatments, and the specifics of supporting client self-management of hypertension, hyperlipidemia and congestive heart failure.

    Self Management of Chronic Disease: Diabetes
    (30 hours)
    This course enhances the LPN knowledge of the pathology, assessments, associated diagnostic tests, pharmacological management current treatments, and the specifics of supporting client self management diabetes.
